Name of the Patient : Abc XyzNevrlmn / F / 67 yrs.
Referred by : Dr. Abc Xyz Shetty.
Examination : M.R.I. of the Brain.

CLINICAL PROFILE :

C/O giddiness with pain in the LUE and LLE since 00.00.0000.
Also C/O loss of taste and smell since 2-3 years.
Known hypertensive. On Rx.

EXAMINATION :

M.R.I of the brain was performed using the following parameters :

5 mm thick T1 Weighted, proton and T2 Weighted axial images.

5 mm thick FLAIR coronal images.

MR cisternogram was obtained in the coronal plane.

OBSERVATION :

A hyperintense area is seen in the left frontal white matter on the proton, T2 Weighted and FLAIR images and which appears hypointense on the T1 Weighted images and is probably ischemic in etiology. Small similar foci are also seen in the centrum semiovale bilaterally.

Lacunar infarcts are noted in the left lentiform nucleus, extending into the left corona radiata and in the centrum semiovale bilaterally.

Prominent perivascular spaces are noted in the posterior parietal periventricular regions, lentiform nuclei and centrum semiovale, bilaterally.

The seventh and eighth cranial nerve complex on either side is unremarkable.





There is mild fullness of both the lateral ventricles more marked on the left side. The third and the fourth ventricles are normal. There is slight prominence of the cerebellar folia bilaterally. The basal cisternal spaces are unremarkable. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.

Inflammatory changes are noted in the ethmoidal air cells, maxillary sinuses and the right mastoid air cells.

IMPRESSION :

1. Ischemic changes in the left frontal region and centrum semiovale bilaterally.

2. Lacunar infarcts in the left lentiform nucleus, extending into the left corona radiata and in the centrum semiovale bilaterally.

3. Right mastoiditis.
