Name of the Patient : Abc XyzBlmn / F / 68 yrs.
Referred by : Dr. Abc Xyz. Kapadia.
Examination : M.R.I. of the Right Knee Joint.
CLINICAL PROFILE :
Alleged H/O fall on 00.00.0000, with pain in the medial aspect of the right knee joint with slight swelling.
EXAMINATION :
M.R.I of the right knee joint was performed using the following parameters :
4 mm thick T1 Weighted, proton and GRASS sagittal images.
4 mm thick T1 Weighted and STIR coronal images.
4 mm thick T1 Weighted GRASS axial images.
OBSERVATION :
Menisci :
There is a curvilinear hyperintense signal on all the pulse sequences in the posterior horn of the medial meniscus reaching upto the articular surface and represents a Grade III meniscal signal (meniscal tear). There also appears to be a tear of the anterior horn of the medial meniscus.
The anterior and posterior horns of the lateral meniscus show a hyperintense signal on all the pulse sequences.
Cruciate Ligaments :
The anterior cruciate ligament is not well identified in its entirety and is probably torn.
The posterior cruciate ligament shows normal contour and signal characteristics.
Collateral Ligaments and the Patellar Tendon :
The medial and lateral collateral ligaments and the patellar tendon are normal.
Hoffas Fat Pad :
The Hoffas fat pad shows irregularly defined areas of hypointensity on the T1 Weighted images. The posterior aspect of the Hoffas pad is irregularly defined.
Articular cartilage and bones :
There is a fracture of the medial condyle of the right femur. Hypointense areas on the T1 Weighted images are seen in the medial condyle which are seen to turn hyperintense on the STIR, T2 Weighted and GRASS images and would represent edema/contusion.
Hypointense areas are also seen in the posterior aspect of the medial tibial condyle on the T1 Weighted images which is seen to turn hyperintense on the T2 Weightged and STIR images. There is disruption of the cortex suggestive of a fracture.
Degenerative changes are noted in the right knee joint with femoral, tibial and patellar osteophytic lipping.
Fluid is seen in the right knee joint and the suprapatellar bursa. Fluid-fluid levels are noted and may represent hemorrhage in the given clinical setting.
IMPRESSION :
1. Grade III meniscal signal (meniscal tear) in the anterior and posterior horns of the medial meniscus of the right knee joint.

2. Tear of the anterior cruciate ligament.
3. Fracture of the medial condyle of the right femur with altered signal which would represent edema/contusion.
4. Altered signal in the posterior aspect of the medial tibial condyle with disruption of the cortex is suggestive of a fracture.
5. Osteophytic lipping in the femur, tibia and patellar (osteoarthritic changes).
6. Fluid in the right knee joint and the suprapatellar bursa with fluid-fluid levels which may represent hemorrhage in the given clinical setting of trauma.
