Name of the Patient : Abc Xyzi lmn / F / 86 yrs.
Referred by : Dr. Abc Xyz Mehta.
Examination : M.R.I. of the Brain.
CLINICAL PROFILE :
H/O sudden onset of jerky movements of BLE with loss of consciousness since 2 am of 00.00.00.
EXAMINATION :
M.R.I of the brain was performed using the following parameters :
5 mm thick T1 Weighted, proton and T2 Weighted axial images.
5 mm thick FLAIR and Fast Scan (T2 *) coronal images.
FEW IMAGES SHOW PATIENT MOTION.
OBSERVATION :
There are ill-defined hyperintense areas on the proton and T2 Weighted images in the periventricular white matter bilaterally, in the subcortical white matter in the fronto-parietal regions bilaterally, pons, thalamus and in the lentiform nuclei bilaterally. These lesions appear iso to hypointense to normal white matter on the T1 Weighted images.
There is mild dilatation of both the lateral and third ventricles. The fourth ventricle is normal.
The basal cisternal spaces are unremarkable. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.
IMPRESSION :
1. Altered signal in the periventricular white matter bilaterally, in the subcortical white matter in the fronto-parietal regions bilaterally, pons, thalamus and in the lentiform nuclei bilaterally most likely represents ischemic changes.
2. Mild age related dilatation of both the lateral and third ventricles.
