Name of the Patient : Abc Xyzsingh lmn / M / 52 yrs.
Referred by : Dr. Abc Xyzvade / Dr. Abc Xyzdar.
Examination : M.R.I. of the Brain.

CLINICAL PROFILE :

Alleged H/O RTA with giddiness.

EXAMINATION :

M.R.I. of the brain was performed using the following parameters:

5 mm thick T1 Weighted, proton and T2 Weighted axial images.

5 mm thick FLAIR and 7 mm thick Fast Scan (T2 *) coronal images.

5 mm thick T1 Weighted sagittal images.

Motion artifacts are noted inspite of sedation.

OBSERVATION :

There is a focal hypointense signal on the proton and T2 Weighted images in the right temporal cortex. This lesion appears nearly iso to slightly hyperintense on the T1 Weighted images and blooms on the Fast Scan (T2 *) images suggesting an acute to early subacute hematoma (there is a remote possibility of this hematoma being extracerebral). This hematoma measures approximately 1.5 x 2.5 x 2.0 cms. There is an ill-defined hyperintense signal on the proton, T2 Weighted and FLAIR images in the right temporal lobe overlying the right temporal hematoma. This signal appears iso to hypointense on the T1 Weighted images and may represent contused brain tissue/edema.

A focal, small, acute hematoma with perilesional edema is also noted in the left temporal cortex, infero-medially (se/im:105/14).

Ill-defined hyperintense signal on the FLAIR images is noted in some of the sulcal spaces in the temporo-occipital lobes bilaterally, along the right tentorial leaflet, in the right high fronto-parietal parafalcine region and in the left temporal region. This may suggest subarachnoid haemorrhage, in the given clinical setting of trauma.

A sliver of subdural fluid/haemorrhage is noted overlying the fronto-temporal lobes bilaterally and right fronto-parietal lobe (parafalcine region).

Suspicious fluid levels in the occipital horns of the lateral ventricles bilaterally, best appreciated on the proton density images may represent intraventricular haemorrhage.

Focal, symmetric, hypointense signal on the Fast Scan images in the globus pallidus may represent calcification.

Mild fullness of the fourth ventricle is noted. There is no shift of the midline structures.

Subgaleal hematoma/edema is noted in the fronto-temporo-parietal regions bilaterally.

IMPRESSION :

1. An approximately 1.5 x 2.5 x 2.0 cms. sized acute to early subacute hematoma in the right temporal cortex (?? extracerebral) with contused brain tissue/edema in the overlying brain parenchyma.

2. A much smaller acute hematoma with perilesional edema in the left temporal cortex, infero-medially.

3. Altered signal in some of the sulcal spaces in the temporo-occipital lobes bilaterally, along the right tentorial leaflet, in the right high fronto-parietal parafalcine region and in the left temporal region may suggest subarachnoid haemorrhage, in the given clinical setting of trauma.

4. A sliver of subdural fluid/haemorrhage overlying the fronto-temporal lobes bilaterally and the right fronto-parietal lobes (parafalcine region).

5. Subgaleal hematoma/edema in the fronto-temporo-parietal regions bilaterally.
