EXAMINATION :
M.R.I. of the brain was performed using the following parameters:
5 mm thick T1 Weighted, proton and T2 Weighted axial images.
5 mm thick FLAIR and 7 mm thick Fast Scan (T2 *) coronal images.
5 mm thick T1 Weighted sagittal images.
OBSERVATION :
There are ill-defined areas in the cerebral peduncles bilaterally which are near isointense to CSF on all the pulse sequences. Hyperintense areas on the proton, T2 Weighted and FLAIR images are seen adjacent to the above lesions and may represent gliotic changes.
A similar focus is noted in the right thalamus.
Hyperintense areas on the proton, T2 Weighted and FLAIR images in the posterior parietal white matter may represent areas of terminal myelination.
There is mild fullness of both the lateral and third ventricles. The fourth ventricle is normal. The basal cisternal spaces are unremarkable. There is no shift of the midline structures. No obvious vascular anomaly is identified on this scan.
IMPRESSION :
The MRI features are suggestive of areas of cystic encephalomalacia in the cerebral peduncles bilaterally and in the right thalamus, the sequelae of previous ADEM.
As compared to the previous MRI dated 00.00.0000 [Study No.00000], areas of cystic encephalomalacia are now seen in the cerebral peduncles and right thalamus. The lesions in the pons, thalami, globus pallidi and the periaqueductal grey matter have resolved.
