Name of the Patient : Abc XyzShlmn / F / 50 yrs.
Referred by : Dr. Abc Xyzq.
Examination : M.R.I. of the Lumbo-sacral Spine.

CLINICAL PROFILE :

C/O backache since 1 year.

EXAMINATION :

M.R.I. of the lumbo-sacral spine was performed using the following parameters :

5 mm thick T1 Weighted and T2 Weighted sagittal images.

5 mm thick T1 Weighted and T2 Weighted axial images.

OBSERVATION :

The lumbar intervertebral discs show loss of water content.

There is a posterior disc herniation at the L5-S1 level with anterior indentation of the thecal sac and bilateral neural foraminal narrowing. Mild ligamentum flavum hypertrophy is also noted at this level.

A posterior disc bulge is noted at the L4-l5 level, with slight ligamentum flavum hypertrophy at that level.

The lumbar facet joints are slightly hypertrophied. The D9-D10, D10-D11 and D11-D12 facet joints are also hypertrophied, more pronounced at the D11-12 level on the right.

The pedicles of the lumbar vertebrae appear congenitally short in their antero-posterior dimensions.

Type II degenerative and fatty marrow changes are see in the lumbar region. A hemangioma with fat content is noted in the L2 vertebral body on the left.

The visualized pre and paravertebral soft tissues are unremarkable.

The conus medullaris terminates at the L2 level and the thecal sac terminates at the S2 level.

The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :

12.0 mm at L1-L2
11.0 mm at L2-L3
11.0 mm at L3-L4
10.0 mm at L4-L5
6.0 mm at L5-S1.

IMPRESSION :

1. A posterior disc herniation at the L5-S1 level with mild ligamentum flavum hypertrophy at this level.

2. A posterior disc bulge at the L4-L5 level with mild ligamentum flavum hypertrophy at this level.

3. Facetal hypertrophy in the lumbar region and at the
D9-D10, D10-D11 and D11-D12 levels.

4. Congenitally short pedicles of the lumbar vertebrae in their antero-posterior dimensions.

5. Slight lumbar canal at the D11-D12, L4-L5 and L5-S1 levels.
