Name of the Patient : Abc Xyz B. lmn / M / 66 yrs.
Referred by : Dr. Abc Xyzhah.
Examination : M.R.I. of the Cervical Spine.
CLINICAL PROFILE :
C/O neck pain radiating to the LUE with paresthesias.
EXAMINATION :
M.R.I. of the cervical spine was performed using the following parameters :
4 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and Fast Scan (T2 *) axial images.
The dorsal spine was screened with 4 mm thick T1 and T2 Weighted sagittal images and the lumbar spine was screened with 5 mm thick T2 Weighted sagittal images.
OBSERVATION :
There is slight retroplacement of the C3 over C4, C5 over C6 and C6 over C7 vertebrae. The C5-C6 and C6-C7 intervertebral discs are reduced in height.
There is a posterior and left postero-lateral disc herniation with peridiscal osteophytes at the C5-C6 level with left neural foraminal narrowing and indentation on the traversing left C7 nerve root.
Posteriorly herniated discs with peridiscal osteophytes are noted at the C3-C4 and C6-C7 levels indenting the dural theca anteriorly.
A left paracentral disc protrusion is seen at the C4-C5 level indenting the anterior aspect of the spinal cord.
Degenerative changes of the joints of Luschka are noted at the C5-C6 level bilaterally and at the C6-C7 level on the left.
Slight ligamentum flavum hypertrophy is noted at the C3-C4 and C6-C7 levels and slight facetal hypertrophy is noted at the C3-C4, C5-C6 and C6-C7 levels.
The cervical vertebral bodies show spotty fatty marrow changes. Type I degenerative marrow change is seen adjacent to the C3-C4 intervertebral disc on the left and Type II degenerative changes are seen at the C5-C6 level.
The pre and paravertebral soft tissues are unremarkable. Anterior peridiscal osteophytes are seen in the cervical region.
The cervical spinal cord reveals normal signal intensity.
The atlanto-axial region and the cervico-medullary junction are unremarkable.
Screening images of the dorsal spine reveal spotty fatty marrow changes in the dorsal vertebrae. Slight anterior wedging of the D6, D9 and D10 vertebrae are noted. Anterior peridiscal osteophytes are seen in the dorsal region.
Screening images of the lumbar spine reveal small postero-central disc herniations at the L2-L3, L3-L4 and L4-L5 levels.

IMPRESSION :
1. Slight retroplacement of the C3 over C4, C5 over C6 and C6 over C7 vertebrae.
2. A posterior and left postero-lateral disc herniation with peridiscal osteophytes at C5-C6 level with left neural foraminal narrowing and indentation on the traversing left C7 nerve root.
3. Posteriorly herniated discs with peridiscal osteophytes at the C3-C4 and C6-C7 levels.
4. Degenerative changes of the joints of Luschka at the C5-C6 level bilaterally and at the C6-C7 level on the left.
5. Spotty fatty marrow changes in the visualized dorsal and cervical vertebral bodies suggests osteoporosis.
