Name of the Patient : Abc Xyz C. Kaplmn / M / 53 yrs.
Referred by : Dr. Abc Xyzhatt / Dr. Abc Xyzngsarkar.
Examination : M.R.I. of the Dorso-lumbar Spine.

CLINICAL PROFILE :

H/O Ileocaecal Kochs.

EXAMINATION :

M.R.I. of the dorso-lumbar spine was performed using the following parameters :

5 mm thick T1 Weighted and T2 Weighted sagittal images.

5 mm thick T1 Weighted and T2 Weighted axial images.

The lumbar spine was scanned with 5 mm thick T1 Weighted sagittal images and 5 mm thick T2 Weighted axial images.

The anterior abdominal wall in the region of interest was scanned with 6 mm thick T1 Weighted and T2 Weighted (with fat saturation) axial images.

OBSERVATION :

There is loss of water content of the most of the visualized dorsal intervertebral discs.

Postero-central protruded discs with peridiscal osteophytes are noted at the D5-D6, D6-D7 and D8-D9 levels.

A left paracentral disc herniation with peridiscal osteophytes is noted at the D10-D11 level, minimally indenting the dorsal spinal cord at that level.


Mild facetal hypertrophy is seen at the D9-D10, D10-D11, D11-D12 and D12-L1 levels.

Anterior and lateral peridiscal osteophytes are noted in the lower dorsal region.

The visualized dorso-lumbar vertebral bodies and the remaining intervertebral discs reveal normal signal intensity. The visualized pre and paravertebral soft tissues are unremarkable.

The visualized lower dorsal spinal cord reveals normal signal intensity.

The conus medullaris terminates at the L1-L2 level.

The lumbar spine images reveal a postero-central and left postero-lateral disc bulge with peridiscal osteophytes at the L5-S1 level, with left neural foraminal narrowing.

A small posterior disc bulge is noted at the L4-L5 level. Posterior peridiscal osteophytes are seen at the L1-L2 and L2-L3 levels.

Facetal and ligamentum flavum hypertrophy is noted at the L4-L5 and L5-S1 levels. Mild facetal hypertrophy is also seen in the rest of the lumbar region.

Vitamin E capsules were placed as markers at the site of the suspected swelling on the anterior abdominal wall on the left. No obvious mass lesion is noted in the anterior abdominal wall at the suspected site of swelling. A suspicious hypointense area is seen in the medial aspect of the spleen on the T1 Weighted images and may be artifactual (However an abdominal ultrasound may be undertaken).

IMPRESSION :

1. Postero-central protruded discs with peridiscal osteophytes at the D5-D6, D6-D7 and D8-D9 levels.

2. A left paracentral disc herniation with peridiscal osteophytes at the D10-D11 level.

3. A postero-central and left postero-lateral disc bulge with peridiscal osteophytes at the L5-S1 level.

4. A small posterior disc bulge at the L4-L5 level.

5. Facetal and ligamentum flavum hypertrophy at the L4-L5 and L5-S1 levels.

6. Mild facetal hypertrophy at the D9-D10, D10-D11, D11-D12 and D12-L1 levels.
