Referred by : Dr. Abc Xyzah.
Examination : M.R.I. of the Cervical Spine.
CLINICAL PROFILE :
C/O paresthesias in BUE with weakness and wasting of the RUE.
EXAMINATION :
M.R.I. of the cervical spine was performed using the following parameters :
4 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and Fast Scan (T2 *) axial images.
OBSERVATION :
There is loss of normal cervical lordosis. Also seen is clockwise rotational anomaly of the cervical vertebrae. There is loss of water content of the cervical intervertebral discs and the C5-C6 and C6-C7 intervertebral discs are decreased in height. Calcium/vacuum phenomena are noted within a few of these intervertebral discs.
There are posterior disc herniations with peridiscal osteophytes indenting on the cord at the C2-C3, C3-C4, C4-C5, C5-C6, C6-C7 and D2-D3 levels. A posterior disc bulge is noted at the C7-D1 level.
Posterior peridiscal osteophytes are seen over the C2-C3 to C7-D1 levels and at the D2-D3 level.

The cervical spinal cord over the C2-C3 to C4-C5 level reveals a subtle hyperintense signal on the T2 Weighted images and these may represent cord edema/ischemia/gliosis. The spinal cord over the C3 to C7 levels appears a little smaller than normal.
The joints of Luschka over the C3-C4 to C6-C7 levels reveal degenerative changes with resultant bilateral neural foraminal narrowing. The C2-C3 to C5-C6 facet joints show degenerative changes.
Mild ligamentum flavum hypertrophy is noted at the C3-C4 level.
The posterior longitudinal ligament over the C3-C4 to C7-D1 levels appears slightly thickened (? ossified).
Anterior disc herniations with peridiscal osteophytes are noted over the C3-C4 to the C7-D1 levels.
The esophagus over the C6 to D1 levels appears larger than normal and may require further evaluation.
The cervical vertebral bodies show fatty marrow changes.
The atlanto-axial region and the cervico-medullary junction are unremarkable.
IMPRESSION :
1. Posterior disc herniations with peridiscal osteophytes at the C2-C3, C3-C4, C4-C5, C5-C6, C6-C7 and D2-D3 levels.
2. A posterior disc bulge at the C7-D1 level.

3. Posterior peridiscal osteophytes over the C2-C3 to C7-D1 levels and at the D2-D3 level.
4. Altered signal of the cervical spinal cord over the C2-C3 to C4-C5 level may represent cord edema/ischemia/gliosis. The spinal cord over the C3 to C7 levels appears a little smaller than normal.
5. Degenerative changes of the joints of Luschka over the C3-C4 to C6-C7 levels.
6. Facetal arthropathy at the C2-C3 to C5-C6 levels.
7. Mild ligamentum flavum hypertrophy at the C3-C4 level.
8. Thickened posterior longitudinal ligament over the C3-C4 to C7-D1 levels (? ossified).
9. The esophagus over the C6 to D1 levels appears larger than normal and may require further evaluation.
10. Tight canal over the C3-C4 to C6-C7 levels maximum at the C3-C4 level.

EXAMINATION :
The lumbar spine was screened as requested with 5 mm thick T1 Weighted and T2 Weighted sagittal images and 5 mm thick T2 Weighted axial images.
OBSERVATION :
There are diffuse posterior disc herniations with peridiscal osteophytes at the L2-L3, L3-L4 and L4-L5 levels with anterior compression of the thecal sac and bilateral neural foraminal narrowing. The facet joints at these levels show hypertrophic degenerative changes with resultant central and lateral canal stenosis.
A posteriorly bulging disc with peridiscal osteophytes is noted at the L1-L2 level with anterior indentation of the thecal sac and bilateral neural foraminal narrowing. A postero-central disc herniation is identified at the L5-S1 level.
The D12-L1, L1-L2 and the L5-S1 facet joints also show degenerative changes.
The pedicles of the lumbar vertebrae appear congenitally short in their antero-posterior dimensions.
Anterior disc herniations are noted over the L1-L2 to the L4-L5 levels. Anterior peridiscal osteophytes are also noted at these levels.
Type II degenerative marrow changes are seen in the lumbar vertebral bodies adjacent to the intervertebral discs. The lumbar intervertebral discs show loss of water content.

The conus medullaris terminates at D12 level and thecal sac terminates at S1 level.
The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :
11.0 mm at L1-L2
9.0 mm at L2-L3
9.0 mm at L3-L4
9.0 mm at L4-L5
8.0 mm at L5-S1.
IMPRESSION :
1. Diffuse posterior disc herniations with peridiscal osteophytes at the L2-L3, L3-L4 and L4-L5 levels with hypertrophic facetal arthropathy and central and lateral canal stenosis.
2. A posteriorly bulging disc with peridiscal osteophytes at the L1-L2 level.
3. A postero-central disc herniation at the L5-S1 level.
4. Congenitally short pedicles of the lumbar vertebrae in their antero-posterior dimensions.
5. Lumbar canal stenosis.
