Name of the Patient : Abc Xyzh lmn / M / 59 yrs.
Referred by : Dr. Abc Xyzah / Dr. Abc Xyzagwati.
Examination : M.R.I. of the Lumbo-sacral Spine.
CLINICAL PROFILE :
C/O backache with radiation of pain to BLE with paresthesias.
EXAMINATION :
M.R.I of the lumbo-sacral spine was performed using the following parameters :
5 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
The L3-L4 and L4-L5 intervertebral discs show loss of water content.
A small posterior disc herniation with peridiscal osteophytes is seen to indent the thecal sac and narrow both neural foramina with indentation upon the exiting L4 nerve roots at the L4-L5 level. The L4-L5 facet joints show hypertrophic degenerative changes. There is ligamentum flavum hypertrophy at the L4-L5 and L5 levels with resultant canal stenosis.
Posteriorly bulging discs are noted at the L3-L4 and L5-S1 levels. The facet joints at these levels show mild degenerative changes.
There are bilateral far lateral disc bulges at the L3-L4 and L4-L5 levels.
An anterior disc herniation with peridiscal osteophytes is noted at the L1-L2 level.

The lumbar vertebral bodies reveal normal signal intensity.
A hemangioma with fat content is noted in the D12 body. The rest of the facet joints and the visualized pre and paravertebral soft tissues are unremarkable.
The conus medullaris terminates at the L1 level and the thecal sac terminates at the S2 level.
The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :
13.0 mm at L1-L2
12.0 mm at L2-L3
12.0 mm at L3-L4
9.0 mm at L4-L5
11.0 mm at L5-S1.
IMPRESSION :
The MRI features are suggestive of :
1. A small posterior disc herniation with peridiscal osteophytes and hypertrophic facetal arthropathy at the L4-L5 level.
2. Ligamentum flavum hypertrophy at the L4-L5 and L5 levels with resultant canal stenosis.
