Name of the Patient : Abc Xyza Kasam Slmn / M / 65 yrs.

Examination : M.R.I. of the Lumbo-sacral Spine.
CLINICAL PROFILE :
C/O backache with pain radiating to BLE with paresthesias.
EXAMINATION :
M.R.I of the lumbo-sacral spine was performed using the following parameters :
5 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
There is scoliosis of the lumbar spine with convexity to the left.
There is slight forward translation of the L3 vertebra over the L4 vertebra. Mild retroplacement of the L5 vertebra over the S1 vertebra is noted.
There is loss of water content of the lumbar intervertebral discs.
There are posteriorly herniated discs with peridiscal osteophytes at the L4-L5 and L5-S1 levels with bilateral neural foraminal narrowing.
There are bilateral postero-lateral and far lateral disc herniations with peridiscal osteophytes at the L3-L4 level with bilateral neural foraminal narrowing and indentation on the exiting L3 nerve roots.

Left far lateral disc bulges are noted at the L1-L2 and L2-L3 levels and bilateral far lateral disc bulges are noted at the L4-L5 and L5-S1 levels.
Severe hypertrophic facetal arthropathy is noted at the L3-L4 and L4-L5 levels bilaterally. Hypertrophic facetal arthropathy is noted at the L1-L2, L2-L3 and L5-S1 levels. Slight ligamentum flavum hypertrophy is noted over the L4-L5 to the L5-S1 level.
Anterior disc herniations with peridiscal osteophytes are noted at the L2-L3, L3-L4 and L4-L5 levels.
The lumbar vertebral bodies reveal normal signal intensity. The visualized pre and paravertebral soft tissues are unremarkable.
The conus medullaris terminates at the L1 level and the thecal sac terminates at the S2 level.
The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :
14.0 mm at L1-L2
13.0 mm at L2-L3
10.0 mm at L3-L4
9.0 mm at L4-L5
11.0 mm at L5-S1.
IMPRESSION :
1. Scoliosis of the lumbar spine with convexity to the left.
2. Posteriorly herniated discs with peridiscal osteophytes at the L4-L5 and L5-S1 levels.

3. Bilateral postero-lateral and far lateral disc herniations with peridiscal osteophytes at the L3-L4 level with indentation on the exiting L3 nerve roots.
4. Severe hypertrophic facetal arthropathy at the L3-L4 and L4-L5 levels bilaterally and Hypertrophic facetal arthropathy at the L1-L2, L2-L3 and L5-S1 levels.
5. Ligamentum flavum hypertrophy over the L4-L5 to the L5-S1 level.
6. Canal stenosis at L3-L4 and L4-L5 levels.
