Name of the Patient : Abc Xylmn / M / 40 yrs.
Referred by : Dr. Abc Xyz. Shah.
Examination : M.R.I. of the Dorso-lumbar Spine.

CLINICAL PROFILE :

C/O backache with pain radiating to the LLE.

EXAMINATION :

M.R.I of the dorso-lumbar spine was performed using the following parameters :

5 mm thick T1 Weighted and T2 Weighted sagittal images.

5 mm thick T1 Weighted and T2 Weighted axial images.

OBSERVATION :

There is loss of water content of the visualized dorso-lumbar intervertebral discs.

There is slight anterior wedging of the D12 and L2 vertebral bodies with focal fatty marrow changes.

There is a fairly large, posteriorly herniated disc with peridiscal osteophytes at the D12-L1 level with compression of the tip of the conus medullaris. The tip of the conus medullaris shows a hyperintense signal on the T2 Weighted images which may represent cord edema/ischemia/contusion.

There is a right postero-lateral disc herniation with peridiscal osteophytes at the L1-L2 level with right neural foraminal narrowing.

A small left paracentral disc herniation is seen at the L2-L3 level.



A small posterior disc bulge is noted at the L3-L4 level.

Slight facetal hypertrophy is seen at the D9-D10 and D10-D11 levels bilaterally and the L2-L3 level on the right.

The rest of the visualized dorso-lumbar vertebral bodies show preponderance of hematopoeitic marrow. The remaining facet joints and the visualized pre and paravertebral soft tissues are unremarkable.

The conus medullaris terminates at the D12-L1 level.

Screening, T1 Weighted sagittal images of the lumbar spine do not reveal any other significant feature of note.

IMPRESSION :

1. Slight anterior wedging of the D12 and L2 vertebral bodies with focal fatty marrow changes, may be the sequelae of previous trauma.

2. A fairly large, posteriorly herniated disc with peridiscal osteophytes at the D12-L1 level with compression of the tip of the conus medullaris which shows altered signal, suggestive of cord edema/ischemia/contusion.

3. A right postero-lateral disc herniation with peridiscal osteophytes at the L1-L2 level.

4. A small left paracentral disc herniation at the L2-L3 level.

5. Slight facetal hypertrophy at the D9-D10 and D10-D11
levels bilaterally and the L2-L3 level on the right.
