Name of the Patient : Abc Xyzvi Sanlmn / F / 72 yrs.
Referred by : Dr. Abc Xyzmpat.
Examination : M.R.I. of the Lumbo-sacral Spine.
CLINICAL PROFILE :
C/O backache with pain radiating to BLE (left more than right).
EXAMINATION :
M.R.I. of the lumbo-sacral spine was performed using the following parameters :
5 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
There is slight scoliosis of the lumbar spine with convexity to the right with slight anti-clockwise rotational anomaly of the mid lumbar vertebrae.
There is loss of water content of the lumbar intervertebral discs.
There is probable sacralization of the L5 vertebra which is as marked on the film. (Please correlate with plain radiographs).
There is a small, posterior disc herniation with peridiscal osteophytes at the L4-L5 level with slight bilateral neural foraminal narrowing.
Small, right far lateral disc herniations with peridiscal osteophytes are seen at the L2-L3 and L3-L4 levels with minimal indentation on the extraforaminal segment of the right L2 and L3 nerve roots, respectively.

Facetal hypertrophy is noted in the lumbar region with ligamentum flavum hypertrophy at the L3, L4 and L5 vertebral levels.
The lumbar vertebral bodies show spotty fatty marrow changes with slight bulging of the intervertebral discs into the bodies of the lumbar vertebrae. Slight anterior wedging of the D11 and D12 vertebral bodies is also noted.
The visualized pre and paravertebral soft tissues are unremarkable.
The conus medullaris terminates at the L1 level and the thecal sac terminates at the S2 level.
The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :
17.0 mm at L1-L2
18.0 mm at L2-L3
16.0 mm at L3-L4
13.0 mm at L4-L5
12.0 mm at L5-S1.
Slight atrophy of the paraspinal muscles is noted in the lumbo-sacral region.
Screening, T2 Weighted sagittal images of the dorsal spine reveal slight anterior wedging of the mid dorsal vertebrae. Small posterior disc herniation with peridiscal osteophytes is noted at the D7-D8 level with slight facetal hypertrophy at that level. The dorsal spinal cord at the D7 and D8 vertebral levels shows a hyperintense signal on the T2 Weighted images suggestive of cord edema/ischemia/gliosis.
Small postero-central protruded discs are noted in the upper dorsal region with slight facetal hypertrophy in the dorsal region.

Screening, T2 Weighted sagittal images of the cervical spine reveal slight facetal hypertrophy in the upper cervical region.
IMPRESSION :
1. Slight scoliosis of the lumbar spine with convexity to the right with slight anti-clockwise rotational anomaly of the mid lumbar vertebrae.
2. Probable sacralization of the L5 vertebra. (Please correlate with plain radiographs).
3. A small, posterior disc herniation with peridiscal osteophytes at the L4-L5 level.
4. Small, right far lateral disc herniations with peridiscal osteophytes at the L2-L3 and L3-L4 levels with minimal indentation on the extraforaminal segment of the right L2 and L3 nerve roots, respectively.
5. Facetal hypertrophy in the lumbar region with ligamentum flavum hypertrophy at the L3, L4 and L5 vertebral levels.
6. Tight lumbar canal at the L3-L4 and L4-L5 levels.
7. Cord edema/ischemia/gliosis at the D7 and D8 vertebral levels.
