Name of the Patient : Abc Xyzlmn / F / 70 yrs.
Referred by : Dr. Abc Xyzshi.
Examination : M.R.I. of the Brain.
CLINICAL PROFILE :
H/O giddiness with fall 2 days back.
Known hypertensive.
EXAMINATION :
M.R.I of the brain was performed using the following parameters :
5 mm thick T1 Weighted, proton and T2 Weighted axial images.
5 mm thick FLAIR and Fast Scan (T2 *) coronal images.
5 mm thick T1 Weighted sagittal images.
OBSERVATION :
There are ill-defined, hyperintense areas on the proton, T2 Weighted and Flair images in the external capsular regions, corona radiata, centrum semiovale, periventricular deep white matter and the thalami bilaterally. These are hypointense to normal white matter on the T1 Weighted images and are most likely ischemic in etiology. Similar subtle altered signal is also noted in the pons.
There is no extradural/subdural collection identified on this study.
There is fullness of both the lateral ventricles. The third and fourth ventricles are normal. Slight prominence of the cerebral cortical sulci is noted.
The basal cisternal spaces are unremarkable. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.
Hyperintense signal is seen within the subgaleal soft tissues in the frontal region on the T2 Weighted images and is of intermediate signal on the T1 Weighted images and represents a subgaleal hematoma in the given clinical setting.
IMPRESSION :
1. Altered signal in the external capsular regions, corona radiata, centrum semiovale, periventricular deep white matter, thalami and in the pons bilaterally is ischemic in etiology.
2. Mild cerebral atrophy.
3. A subgaleal hematoma in the frontal region.
