Examination : M.R.I. of the Right Thigh.
CLINICAL PROFILE :
C/O swelling over the proximal right thigh since 5-6 months.
EXAMINATION :
M.R.I. of the right thigh was performed using the following parameters :
10 mm thick T1 Weighted and T2 Weighted axial images.
7 mm thick STIR and T1 Weighted sagittal images.
10 mm thick T1 Weighted coronal images.
OBSERVATION :
There is replacement of the normal marrow of the proximal shaft of the right femur by a hypointense lesion on the T1 Weighted images which appears hyperintense on the T2 Weighted and STIR images. The affected segment extends from about 3.0 cms distal to the lesser trochanter, for a distance of 16.0 cms inferiorly. There is a sharp zone of transition between the normal and abnormal marrow. Circumferential cortical thickening is noted over the extent of the affected segment of the femoral shaft.
There is a fairly large, intermediate signal intensity mass lesion on the T1 Weighted images in the anterior compartment of the right thigh, extending from about the level of the lesser trochanter, distally for about 13.0 cms. This lesion appears hyperintense on the T2 Weighted images with a necrotic focus, centrally. This lesion is in close relation to the femoral shaft over its extent with resultant displacement of the quadriceps muscles, peripherally. The neurovascular bundle is also displaced antero-medially by the lesion. No vascular encasement is noted.

IMPRESSION :
Altered signal in the proximal femoral shaft with a soft tissue mass lesion in the anterior compartment of the right thigh as described is not specific for a single etiology. This most likely is a neoplastic lesion. A soft tissue sarcoma may be considered in the differential diagnosis.
The possibility of this being an infective process is considered less likely.
