Name of the Patient : Abc Xyz Gajlmn / F / 18 months.
Referred by : Dr. Abc Xyzauhan.
Examination : M.R.I. of the Dorsal Spine.
CLINICAL PROFILE :
C/O fever with loss of appetite since 3-4 months.
EXAMINATION :
M.R.I of the dorsal spine was performed using the following parameters :
3 mm thick T1 Weighted and T2 Weighted sagittal images.
4 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
There is slight collapse of the D8 and D9 vertebral bodies which show hypointense signal on the T1 Weighted images replacing the normal marrow. These are seen to turn heterogeneously hyperintense on the T2 Weighted images. The D8-D9 intervertebral disc is involved with breach in the adjacent cortical endplates. There is pre and paravertebral soft tissue extension over the
D6-D7 to the D9 vertebral levels. Large anterior epidural collection is seen over the D7 to D9-D10 levels with posterior displacement and compression of the spinal cord. The spinal cord however shows normal signal intensity.
The rest of the visualized dorsal vertebral bodies and remaining intervertebral discs reveal normal signal intensity. The facet joints are unremarkable.
The visualized dorsal spinal cord reveals normal signal intensity.
The conus medullaris terminates at the L1 level.

IMPRESSION :
The MRI features are suggestive of collapse of the D8 and D9 vertebral bodies (with altered signal) and involvement of the
D8-D9 intervertebral disc with pre and paravertebral and epidural extension as described, most likely represents a granulomatous infective process like tuberculosis.
The possibility of this lesion being a round cell tumor cannot be entirely ruled out, though less likely.
