Name of the Patient : Abc Xyzh Palmn / M / 20 yrs.
Referred by : Dr. Abc Xyzah.
Examination : M.R.I. of the Brain.

CLINICAL PROFILE :

H/O right sided weakness in December 0000 from which patient has recovered partially.
C/O right sided headaches with giddiness and vomiting since the morning of 00.00.0000.

EXAMINATION :

M.R.I of the brain was performed using the following parameters :

5 mm thick T1 Weighted, proton and T2 Weighted axial images.

4 mm thick FLAIR coronal images.

3 mm thick FLAIR axial images.

MR cisternogram was obtained in the coronal plane.

OBSERVATION :

There is a CSF signal intensity lesion on all the pulse sequences along the anterior margin of the medulla oblongata on the left, medially. This represents a gliotic/encephalomalacic focus, the sequelae of previous vascular insult. Resultant slight volume loss of the left pyramid of the medulla oblongata is noted.

There is no other focal area of altered signal identified in the brain parenchyma.

The seventh and eighth cranial nerve complex is unremarkable on either side.



Both the lateral, third and the fourth ventricles are normal. The basal cisternal spaces are unremarkable. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.

IMPRESSION :

Volume loss of the left pyramid of the medulla oblongata with focal altered signal in that region is the sequelae of previous vascular insult/demyelination. No new lesion is noted.
