Name of the Patient : Abc Xyzshna Palmn / M / 52 yrs.
Referred by : Dr. Abc Xyzah.
Examination : M.R.I. of the Brain.

CLINICAL PROFILE :

C/O gait ataxia and slurred speech.

EXAMINATION :

M.R.I of the brain was performed using the following parameters :

5 mm thick T1 Weighted, proton and T2 Weighted axial images.

4 mm thick T1 Weighted coronal images.

MR cisternogram was obtained in the coronal plane.

5 mm thick T1 Weighted sagittal images.

OBSERVATION :

There is a large extra-axial mass lesion in the right cerebello-pontine angle cistern which measures approximately 6.0 x 5.2 x 3.5 cms. This lesion has a solid component anteriorly which is hypointense on the T1 Weighted images and turns heterogeneously hyperintense on the T2 Weighted images. There is extension into the internal auditory meatus on the right side. A large cystic component is seen posteriorly which is slightly hyperintense to CSF on all the pulse sequences. There is mass effect with compression upon the right lateral aspect of the pons and effacement of the perimedullary and perimesencephalic cistern. There is compression and near complete obliteration of the fourth ventricle with resultant dilatation of both the lateral and third ventricles. The fourth ventricle is shifted to the left.

There is effacement of the cerebral cortical sulci which may suggests raised intracranial tension.

Note is made of an empty sella.

No obvious vascular anomaly is identified on this study.

Incidental note is made of a left maxillary polyp and inflammatory changes in the sphenoid sinus.

IMPRESSION :

The MRI features are suggestive of a large extra-axial mass lesion in the right cerebello-pontine angle cistern measuring approximately 6.0 x 5.2 x 3.5 cms. with a solid and cystic component (? trapped CSF) and probably represents an acoustic neurinoma.
