Name of the Patient : Abc Xyz Hemlmn / M / 36 yrs.
Referred by : Dr. Abc Xyztrak.
Examination : M.R.I. of the Brain.
CLINICAL PROFILE :
Alleged H/O fall at the age of 5 years.
At present has pathologic laughter.
EXAMINATION :
M.R.I of the brain was performed using the following parameters :
5 mm thick T1 Weighted, proton and T2 Weighted axial images.
3 mm thick T2 Weighted coronal images.
5 mm thick FLAIR coronal images.
OBSERVATION :
There is prominence of the cortical sulci in the left cerebral hemisphere more so in the left fronto-temporal region.
Hyperintense areas on the Flair images are seen within the subcortical white matter in the left frontal lobe and these may represent gliotic changes.
There is fullness of the frontal horn of the left lateral ventricle.
There is mild prominence of the cerebellar folia.
The right lateral, third and the fourth ventricles are normal. The basal cisternal spaces are unremarkable. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.

IMPRESSION :
The MRI features are suggestive of atrophy of the left cerebral hemisphere, more so in the left fronto-temporal region with areas of altered signal within the subcortical white matter in the left frontal lobe which may represent gliotic changes.
