Name of the Patient : Abc Xyz B. Klmn / M / 40 yrs.
Referred by : Dr. Abc Xyzah.
Examination : M.R.I. of the Lumbo-sacral Spine.
CLINICAL PROFILE :
C/O backache with occasional paresthesias in BLE since 1 year.
EXAMINATION :
M.R.I of the lumbo-sacral spine was performed using the following parameters :
5 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
There is loss of water content of the lumbar intervertebral discs.
There is a right postero-lateral disc bulge with peridiscal osteophytes at the L5-S1 level with right neural foraminal narrowing.
Posteriorly bulging disc with peridiscal osteophytes is noted at the L4-L5 level with bilateral neural foraminal narrowing.
A small, right paracentral protruded disc with peridiscal osteophytes is noted at the L3-L4 level.
A small, right paracentral disc herniation with peridiscal osteophytes is noted at the L2-L3 level with right neural foraminal narrowing.
Small posterior disc bulge with peridiscal osteophytes is noted at the D12-L1 level.

Slight facetal hypertrophy is noted at the L4-L5 and L5-S1 levels.
A sclerotic focus, which is hypointense on all the pulse sequences is noted in the postero-inferior quadrant of the L1 vertebral body.
The rest of the lumbar vertebral bodies reveal excess of hematopoeitic marrow. The visualized pre and paravertebral soft tissues are unremarkable.
The conus medullaris terminates at the L1-L2 level and the thecal sac terminates at the S2 level.
The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :
14.0 mm at L1-L2
12.0 mm at L2-L3
13.0 mm at L3-L4
13.0 mm at L4-L5
11.0 mm at L5-S1.
IMPRESSION :
1. A right postero-lateral disc bulge with peridiscal osteophytes at the L5-S1 level.
2. Posteriorly bulging discs with peridiscal osteophytes at the L4-L5 level with bilateral neural foraminal narrowing.
3. A small, right paracentral protruded disc with peridiscal osteophytes at the L3-L4 level.

6. Slight facetal hypertrophy at the L4-L5 and L5-S1 levels.
