Name of the Patient : Abc Xyzai Nlmn / F / 50 yrs.
Referred by : Dr. Abc Xyzah.
Examination : M.R.I. of the Cervico-dorsal Spine.
CLINICAL PROFILE :
C/O paraparesis with bladder involvement since 5 days.
EXAMINATION :
M.R.I of the cervico-dorsal spine was performed using the following parameters :
4 mm thick T1 Weighted and T2 Weighted sagittal images.
8 mm thick T1 Weighted and T2 Weighted axial images.
The dorsal spine was screened with 4 mm thick T2 Weighted sagittal images and the brain was screened with 5 mm thick T2 Weighted axial images.
OBSERVATION :
The spinal cord is seen to be swollen and shows a hyperintense signal on the T2 Weighted images, centrally from over the C3 to L1 vertebral levels.
The visualized cervico-dorsal intervertebral discs show loss of water content. A hemangioma with fat content (hyperintense on all the pulse sequences) is seen within the D6 vertebral body.
Note is made of a Ryles tube in the esophagus.
The rest of the visualized cervico-dorsal vertebral bodies reveal normal signal intensity. The facet joints and the visualized pre and paravertebral soft tissues are unremarkable.
The conus medullaris terminates at the L1 level.
Hyperintense areas on the T2 Weighted images are seen within the medulla, pons, left cerebral peduncle, genu of the left internal capsule and the left occipital lobe.
IMPRESSION :
The MRI features are suggestive of altered signal within the spinal cord over the C3 to L1 vertebral levels and this is not specific for a single diagnosis. This may represent :
1. Demyelination.
2. Myelitis.
Areas of altered signal within the brain may suggest acute disseminated encephalomyelitis. A contrast enhanced scan may be worthwhile.
