Name of the Patient : Abc XyzSakhalmn / M / 55 yrs.
Referred by : Dr. Abc Xyzrani.
Examination : M.R.I. of the Brain.
CLINICAL PROFILE :
C/O weakness of the LUE on 00.00.0000 from which patient has recovered.
EXAMINATION :
M.R.I of the brain was performed using the following parameters :
5 mm thick T1 Weighted, proton and T2 Weighted axial images.
5 mm thick FLAIR and Fast Scan (T2 *) coronal images.
5 mm thick T1 Weighted sagittal images.
OBSERVATION :
There is evidence of an approximately 1.7 x 4.0 x 2.6 cms sized well-marginated, hyperintense lesion on the T1 Weighted images in the right lentiform nucleus. This lesion remains hyperintense on the proton, T2 Weighted and FLAIR images and appears relatively hypointense on the Fast Scan (T2 *) images. A peripheral hypointense rim, better appreciated on the Fast Scan (T2 *) images is noted around the lesion. There is perilesional edema, with slight compression upon the right lateral ventricle.
There is mild dilatation of the left lateral ventricle. The third and the fourth ventricles show mild fullness. There is slight prominence of the cerebral cortical sulci, cerebellar folia and basal cisternal spaces bilaterally. There is slight blunting of the right cerebral peduncle. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.
Incidental note is made of an empty sella.

IMPRESSION :
An approximately 1.7 x 4.0 x 2.6 cms sized lesion in the right lentiform nucleus following the signal characteristics of a late subacute hematoma.
No previous investigations were available for comparison.
