Name of the Patient : Abc Xyzmar Blmn / M / 42 yrs.
Referred by : Dr. Abc Xyz.
Examination : M.R.I. of the Lumbo-sacral Spine.

CLINICAL PROFILE :

C/O backache with pain radiating to BLE with paresthesias.

EXAMINATION :

M.R.I. of the lumbo-sacral spine was performed using the following parameters :

5 mm thick T1 Weighted and T2 Weighted sagittal images.

5 mm thick T1 Weighted and T2 Weighted axial images.

OBSERVATION :

Mild retroplacement of the L3 over the L4 and of the L4 over the L5 vertebrae is noted. The D12-L1, L3-L4 and L4-L5 intervertebral discs show loss of water content.

A posterior disc herniation with peridiscal osteophytes is seen to indent the thecal sac and narrow both neural foramina at the L4-L5 level. A disc portion is seen to lie within the right lateral recess of the L5 vertebra with resultant impingement of the traversing right L5 nerve root.

A postero-central disc herniation with peridiscal osteophytes is seen to indent the thecal sac at the L3-L4 level. Also seen is a postero-central disc herniation with peridiscal osteophytes indenting the ventral aspect of the cord at the D11-D12 level.


There is mild facetal hypertrophy at the D11-D12 level and in the lumbar region.

Right postero-lateral and right far lateral disc bulges are noted at the L5-S1 level.

The lumbar vertebral bodies show an inhomogeneous marrow pattern. The visualized pre and paravertebral soft tissues are unremarkable.

The conus medullaris terminates at the L2 level and the thecal sac terminates at the S1 level.

The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :

10.0 mm at L1-L2
17.0 mm at L2-L3
15.0 mm at L3-L4
10.0 mm at L4-L5
11.0 mm at L5-S1.

IMPRESSION :

1. A posterior disc herniation with peridiscal osteophytes at the L4-L5 level. A disc portion is seen to lie within the right lateral recess of the L5 vertebra with resultant impingement of the traversing right L5 nerve root.

2. Postero-central disc herniations with peridiscal osteophytes at the D11-D12 and L3-L4 levels.

3. Mild facetal hypertrophy at the D11-D12 level and in the lumbar region.
