Name of the Patient : Abc Xyzsh Kalmn / M / 12 yrs.
Referred by : Dr. Abc Xyzal.
Examination : M.R.I. of the Brain.

CLINICAL PROFILE :

C/O right zygomatico-orbital neurofibroma since birth.

EXAMINATION :

M.R.I of the brain was performed using the following parameters :

5 mm thick T1 Weighted, proton and T2 Weighted axial images.

5 mm thick FLAIR coronal images.

4 mm thick T1 Weighted coronal images.

OBSERVATION :

There is an ill-defined soft tissue mass lesion along the surface of the skin in the right zygomatic and temporal regions. This lesion is seen to extend in the periorbital region on the right side along the inferior margin of the right orbit. The inferior rectus muscle at its insertion cannot be well-differentiated from the lesion. The inferior wall of the orbit appears to be suspiciously deficient. This lesion is of intermediate signal on the T1 Weighted images and is heterogeneously hypointense on the proton, T2 Weighted and FLAIR images.

A similar lesion is noted in the subgaleal region in the suboccipital region.

There is fullness of both the lateral ventricles. The third and the fourth ventricles are normal.

There is beaking of the cerebellar tonsils with cerebellar tonsillar ectopia and the tip is seen to lie about 1.0 cm inferior to the foramen magnum level.




The basal cisternal spaces are unremarkable. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.

IMPRESSION :

In a known C/O right zygomatico-orbital neurofibroma since birth, the MRI features of :

1. A mass lesion along the surface of the skin in the right zygomatic and temporal regions with extensions as described and another lesion in the subgaleal soft tissues in the suboccipital region.

2. Beaking of the cerebellar tonsils with cerebellar tonsillar ectopia and the tip lying about 1.0 cm inferior to the foramen magnum level.

3. Fullness of both the lateral ventricles.
