Name of the Patient : Abc Xyzalal K. lmn / M / 80 yrs.
Referred by : Dr. Abc Xyzatt / Dr. Abc Xyz Shah.
Examination : M.R.I. of the Brain.

CLINICAL PROFILE :

C/O generalized weakness over a period of time with H/O fall twice within the last 15 days.
Known hypertensive.

EXAMINATION :

M.R.I of the brain was performed using the following parameters :

5 mm thick T1 Weighted, proton and T2 Weighted axial images.
5 mm thick FLAIR and Fast Scan (T2 *) coronal images.
5 mm thick T1 Weighted sagittal images.

OBSERVATION :

There are hyperintense areas on the proton, T2 Weighted and Flair images in the frontal, parietal and periventricular deep white matter. These are hypointense to white matter on the T1 Weighted images and are ischemic in etiology. Similar areas are also noted within the pons.

There is fullness of the ventricular system with prominence of the cerebral cortical sulci and cerebellar folia. The basal cisternal spaces are unremarkable. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.

Incidental note is made of inflammatory changes in the maxillary sinuses bilaterally.

IMPRESSION :

1. Altered signal in the frontal, parietal, periventricular deep white matter and pons are ischemic in etiology.

2. Cerebral and cerebellar atrophy.
