Name of the Patient : Abc Xyz Salmn / M / 45 yrs.
Referred by : Dr. Abc Xyzagh / Dr. Abc Xyzl Jain.
Examination : M.R.I. of the Lumbo-sacral Spine.
CLINICAL PROFILE :
C/O backache with radiation of pain to BLE with paresthesias.
EXAMINATION :
M.R.I of the lumbo-sacral spine was performed using the following parameters :
5 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
There is loss of normal lumbar lordosis. The D12-L1, L2-L3 and L4-L5 intervertebral discs show loss of water content.
A large postero-central disc extrusion is seen to compress the thecal sac with bilateral neural foraminal narrowing at the L4-L5 level. A disc portion is seen to lie within the anterior epidural space at the L5 vertebral level.
A small postero-central disc herniation is seen to indent the thecal sac at the L2-L3 level.
A limbus vertebra is noted at the L4 level. A Schmorls node is seen at the inferior aspect of the L4 vertebral body.
The lumbar vertebral bodies and the remaining intervertebral discs reveal normal signal intensity. The facet joints and the visualized pre and paravertebral soft tissues are unremarkable.
The conus medullaris terminates at the L1 level and the thecal sac terminates at the S1-S2 level.
The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :
15.0 mm at L1-L2
11.0 mm at L2-L3
12.0 mm at L3-L4
8.0 mm at L4-L5
13.0 mm at L5-S1.
IMPRESSION :
The MRI features are suggestive of :
1. A large postero-central disc extrusion with severe canal stenosis at the L4-L5 level.
2. A small postero-central disc herniation at the L2-L3 level.
