Examination : M.R.I. of the Sella & Perisellar Region.
CLINICAL PROFILE :
Serum Prolactin level was 0000.00 ng/ml on 00.00.0000.
EXAMINATION :
M.R.I of the sella and perisellar region was performed using the following parameters :
3 mm thick T1 Weighted, T2 Weighted and FLAIR coronal images.
3 mm thick T1 Weighted sagittal images.
The brain was screened with 5 mm thick T2 Weighted axial images.
OBSERVATION :
There is seen a fairly large well marginated, approximately 2.5 x 1.8 x 1.6 cms sized, predominantly hypointense mass lesion on the T1 Weighted images in the sella. This lesion appears predominantly hyperintense on the T2 Weighted images. The solid component of the lesion is seen along its posterior and inferior margins, whereas a cystic/necrotic component is noted along its anterior and superior quadrant. Part of the normal pituitary gland is seen to be displaced slightly superior and to the right (scan 103.4) and the lesion seems to arise from the inferior margin of the anterior pituitary gland slightly to the left of the midline. There is ballooning of the sella with remodeling of the floor of the sella more so on the left side.
The lesion extends into the suprasellar cistern with minimal indentation on the optic chiasma. The pituitary stalk is minimally displaced to the right. There is no extension of the lesion into the cavernous sinuses.
The posterior pituitary gland reveals normal hyperintense signal on the T1 Weighted images. The hypothalamus is unremarkable.
The T2 Weighted axial images of the brain do not reveal any significant feature of note.
Inflammatory changes are noted in the maxillary sinuses and ethmoidal air cells bilaterally. Septae are seen in the sphenoid sinus.
IMPRESSION :
An approximately 2.5 x 1.8 x 1.6 cms sized mass lesion in the sella with signal characteristics and extensions as described, most likely represents a pituitary macroadenoma.
If clinically indicated a contrast enhanced scan may be worthwhile.
