Name of the Patient : Abc Xyz Chlmn / M / 30 yrs.
Referred by : Dr. Abc Xyzah.
Examination : M.R.I. of the Hip joints.
CLINICAL PROFILE :
C/O pain in the left hip joint since 15 days.
H/O steroid intake for 6 months for SLE with interstitial pneumonia.
EXAMINATION :
M.R.I. of both hips was performed using the following parameters:
5 mm thick T1 Weighted and STIR coronal images.
5 mm thick T1 Weighted and T2 Weighted (with fat saturation) axial images.
5 mm thick Proton density sagittal images.
OBSERVATION :
There is loss of normal contour of the left femoral head.
There are ill-defined hypointense areas on the T1 Weighted images within the left femoral head which appear hyperintense on the T2 Weighted and STIR images. Small hyperintense areas on the T1 Weighted images which turn hypointense on the T2 Weighted and STIR images are also seen. The double line sign is noted. Ill-defined hyperintense areas on the T2 Weighted and STIR images are noted in the neck and upper shaft of the left femur which may represent bone marrow edema. There is slight thinning of the articular cartilage over the left femoral head. The articular cartilage over the left acetabulum is unremarkable. Hyperintense signal on the T2 Weighted and STIR images is seen within the left hip joint suggestive of large effusion.

There is a hyperintense signal on the T1 Weighted images along the antero-superior quadrant of the right femoral head. This is seen to follow fat signal intensity characteristics on all the pulse sequences. The double line sign is noted. Ill-defined hypointense areas on the T1 Weighted images are seen in the
rest of the femoral head and the neck which turn hyperintense on the T2 Weighted and STIR images which may represent edema. The articular cartilage over the right femoral head and the right acetabulum appear normal.
The musculature surrounding the left hip joint appears atrophied as compared to the right.
IMPRESSION :
The MRI features are suggestive of :
1. Altered signal in the left femoral head suggestive of Class A and Class C avascular necrosis (Mitchells classification)
2. Class A avascular necrosis in the right femoral head.
3. Large effusion within the left hip joint.
