Name of the Patient : Abc Xyz Malmn / M / 73 yrs.
Referred by : Dr. Abc Xyzauhan.
Examination : M.R.I. of the Lumbo-sacral Spine.
CLINICAL PROFILE :
C/O backache. Alleged H/O fall 3 weeks ago.
EXAMINATION :
M.R.I of the lumbo-sacral spine was performed using the following parameters :
5 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
There is anterior wedging of the D12 vertebral body. Hypointense areas on the T1 Weighted images which turn subtly hyperintense on the T2 Weighted images are seen within the inferior aspect of the D12 vertebral body and superior aspect of the L2 vertebral body and may represent bone edema. Linear hypointensities on all the pulse sequences seen within the inferior aspect of the D12 vertebral body may represent compressed trabeculae. Slight central wedging of the L5 vertebral body is noted.
The visualized dorso-lumbar vertebral bodies show areas of fatty replacement of normal marrow and which may represent osteoporotic changes. The visualized dorso-lumbar intervertebral discs show loss of water content.
Mild posterior disc bulges are noted at the L4-L5 and L5-S1 levels with indentation upon the thecal sac at the L4-L5 level.
There are bilateral far lateral disc bulges over the L2-L3 to L5-S1 levels.
The L3-L4, L4-L5 and L5-S1 facet joints show mild degenerative changes.
The rest of the facet joints and the visualized pre and paravertebral soft tissues are unremarkable.
The conus medullaris terminates at the D12 level and the thecal sac terminates at the S1 level.
The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :
20.0 mm at L1-L2
18.0 mm at L2-L3
16.0 mm at L3-L4
13.0 mm at L4-L5
13.0 mm at L5-S1.
IMPRESSION :
The MRI features are suggestive of :
1. A compression fracture of the D12 vertebral body superimposed upon an osteoporotic spine with bone edema in the D12 and L2 vertebral bodies.
2. Posterior disc bulges at the L4-L5 and L5-S1 levels.
3. Mild facetal arthropathy at the L3-L4, L4-L5 and L5-S1 levels.
