Name of the Patient : Abc Xyzr Klmn / M / 73 yrs.
Referred by : Dr. Abc Xyzatil.
Examination : M.R.I. of the Brain.
CLINICAL PROFILE :
C/O 2 episodes of seizures on 00.00.0000.
EXAMINATION :
M.R.I of the brain was performed using the following parameters :
5 mm thick T1 Weighted, proton and T2 Weighted axial images.
5 mm thick FLAIR and Fast Scan (T2 *) coronal images.
OBSERVATION :
Lacunar infarcts are noted in the pons on the right, external capsular regions bilaterally, head of caudate nuclei bilaterally and in the corona radiata bilaterally.
There are ill-defined, hyperintense areas on the proton, T2 Weighted images and FLAIR images in the posterior parietal periventricular white matter bilaterally and in the subcortical and deep white matter in the frontal regions bilaterally. These most likely represent ischemic lesions.
Dilated perivascular spaces are seen in the lentiform nuclei, corona radiata and centrum semiovale bilaterally.
There is mild fullness of both the lateral ventricles. The third and the fourth ventricles are normal. There is slight prominence of the cerebral cortical sulci and the basal cisternal spaces bilaterally. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.
There is no obvious haemorrhage on this study.
IMPRESSION :
1. Altered signal in the posterior parietal periventricular white matter bilaterally and in the subcortical and deep white matter in the frontal regions bilaterally most likely represent ischemic lesions.
2. Lacunar infarcts in the pons on the right, external capsular regions bilaterally, head of caudate nuclei bilaterally and in the corona radiata bilaterally.
3. Age related cerebral cortical atrophy.
