Name of the Patient : Abc Xyz Gheelmn / M / 41 yrs.
Referred by : Dr. Abc Xyzra.
Examination : M.R.I. of Both Hips.
CLINICAL PROFILE :
H/O fall on 00.00.0000 with intertrochantric fracture of left femur. ORIF with screw fixation done on 00.00.0000 which was removed on 00.00.0000.
Now C/O pain in both hips (left more than right) with limp with a H/O recent fall 15 days ago.
EXAMINATION :
M.R.I of both hips was performed using the following parameters :
5 mm thick T1 Weighted and STIR coronal images.
5 mm thick T1 Weighted and T2 Weighted (with fat saturation) axial images.
5 mm thick Proton density sagittal images.
OBSERVATION :
There are post-operative changes with susceptibility artifacts
in the soft tissues on the left side adjacent to the
left hip joint.
An ill-defined, hypointense area is seen in the neck of the left femur on the T1 Weighted images which turns hyperintense on the T2 Weighted and STIR images. Susceptibility artifacts are also noted and this represents the track of the previous screw fixation. Old fracture fragments are noted along the antero-inferior margin of the head of the left femur. There is slight irregularity and thinning of the articular cartilage over the head of the left femur. A small degenerative pit is noted in the roof of the left acetabulum. Minimal fluid is noted in the left hip joint. The musculature around the left hip joint are atrophied. There is no definite evidence of an area of avascular necrosis in the head of the left femur, on this study.

The right hip joint is unremarkable.
The visualized pelvis is normal.
IMPRESSION :
The MRI features are suggestive of :
1. Post-operative status.
2. Altered signal in the neck of the left femur is the sequelae of previous screw fixation.
3. Old fractured fragments along the antero-inferior margin of the head of the left femur.
4. Minimal effusion in the left hip joint.
