Name of the Patient : Abc Xyz Blmn / F / 28 yrs.
Referred by : Dr. Abc Xyzo.
Examination : M.R.I. of the Dorso-lumbar Spine.
CLINICAL PROFILE :
C/O backache with paresthesias in the LLE.
H/O mild fever off and on.
EXAMINATION :
M.R.I of the dorso-lumbar spine was performed using the following parameters :
4 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
3 mm thick T2 Weighted coronal images.
The lumbar spine was screened with 5 mm thick T1 Weighted and T2 Weighted sagittal images and the cervical spine was screened with 4 mm thick T2 Weighted sagittal images.
Patient refused contrast administration.
OBSERVATION :
There is evidence of a fairly well-circumscribed lesion having a diameter of approximately 8.0 mms located within the conus medullaries at the D12 vertebral level. It is hypointense with a hyperintense rim on the T2 Weighted images and is not well-identified on the T1 Weighted images.
The conus medullaries is seen to be swollen.
Hyperintense signal on the T2 Weighted images (hypointense on the T1 Weighted images) is seen within the cord over the D4 to L1-L2 levels. It is located within the central portion of the cord.

The D5 vertebral body is hyperintense on the T2 Weighted images and may represent a hemangioma.
The rest of the visualized dorso-lumbar vertebral bodies and the intervertebral discs reveal normal signal intensity. The facet joints and the visualized pre and paravertebral soft tissues are unremarkable.
The conus medullaris terminates at the L1 level.
IMPRESSION :
The MRI features are suggestive of a lesion having a diameter of approximately 8.0 mms within the conus medullaries at the D12 vertebral level with altered cord signal over the D4 to L1-L2 levels. This may represent a granuloma following the signal characteristics of a tuberculoma.
A contrast enhanced scan would be worthwhile.
