Examination : M.R.I. of the Lumbo-sacral Spine.

CLINICAL PROFILE :

C/O backache with pain radiating to BLE (left more than right).

EXAMINATION :

M.R.I. of the lumbo-sacral spine was performed using the following parameters :

5 mm thick T1 Weighted and T2 Weighted sagittal images.

5 mm thick T1 Weighted and T2 Weighted axial images.

OBSERVATION :

The lumbar intervertebral discs show loss of water content.

There is a left paracentral disc extrusion at the L5-S1 level with antero-lateral indentation of the thecal sac. Slight inferior migration of the disc is noted indenting the left S1 nerve root and posteriorly displacing the right S1 nerve root.

There is slight retroplacement of the L3 over the L4 vertebral body with a pseudoposterior disc herniation at the L3-L4 level indenting the anterior aspect of the thecal sac.

A posterior disc bulge is seen at the L3-L4 level.

Schmorls nodes is seen in the inferior aspect of the L2 vertebral body.

The lumbar facet joints show mild degenerative changes. Fatty changes are noted in the lumbar vertebral bodies.

The visualized pre and paravertebral soft tissues are unemarkable.

The conus medullaris terminates at the D12-L1 level and the thecal sac terminates at the S2 level.

The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :

16.0 mm at L1-L2
16.0 mm at L2-L3
15.0 mm at L3-L4
15.0 mm at L4-L5
11.0 mm at L5-S1.

IMPRESSION :

1. A left paracentral disc extrusion at the L5-S1 level with slight inferior migration of the disc indenting the left S1 nerve root and posteriorly displacing the right S1 nerve root.

2. Slight retroplacement of the L3 over the L4 vertebral body with a pseudoposterior disc herniation at the L3-L4 level.

3. Facetal arthropahty in the lumbar region.
