Name of the Patient : Abc Xyzra Kollmn / M / 25 yrs.
Referred by : Dr. Abc Xyzvale.
Examination : M.R.I. of the Brain.

CLINICAL PROFILE :

C/O 2 episodes of seizures with vomiting on 00.00.0000.
H/O fever since 8 days.

EXAMINATION :

M.R.I of the brain was performed using the following parameters :

5 mm thick T1 Weighted, proton and T2 Weighted axial images.

3 mm thick T2 Weighted and 5 mm thick FLAIR coronal images.

OBSERVATION :

There is an ill-defined hypointense area on the T1 Weighted images in the right temporo-parietal region located postero-superiorly to the Sylvian fissure involving the grey and subcortical white matter. This is seen to follow CSF signal intensity characteristics on all the pulse sequences and is in communication with the subarachnoid space. Adjacent areas which are hyperintense on the proton, T2 Weighted and FLAIR images and hypointense to white matter on the T1 Weighted images would represent areas of gliosis. There is slight prominence of the adjacent sulcal spaces in that region and this lesion would represent an area of cystic encephalomalacia.

The hippocampal complex on either side is unremarkable.

Both the lateral, third and the fourth ventricles are normal. The basal cisternal spaces are unremarkable. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.

IMPRESSION :

An area of cystic encephalomalacia in the right temporo-parietal region located postero-superiorly to the Sylvian fissure involving the grey and subcortical white matter, most likely the sequelae of a previous vascular insult.
