Name of the Patient : Abc Xyzhila Talmn / F / 57 yrs.
Referred by : Dr. Abc Xyzrekh.
Examination : M.R.I. of the Lumbo-sacral Spine.

CLINICAL PROFILE :

C/O backache with pain radiating to the LLE with paresthesias.

EXAMINATION :

M.R.I of the lumbo-sacral spine was performed using the following parameters :

5 mm thick T1 Weighted and T2 Weighted sagittal images.

5 mm thick T1 Weighted and T2 Weighted axial images.

OBSERVATION :

There is slight retroplacement of the L3 vertebra over the L4 vertebra.

There is reduction in height of the L1-L2 and L5-S1 discs and loss of water content of the lumbar intervertebral discs.

Posterior and bilateral far lateral disc herniations with peridiscal osteophytes are noted at the L5-S1 level with slight bilateral neural foraminal narrowing.

A posterior and left far lateral disc herniations with peridiscal osteophytes is noted at the L4-L5 level with bilateral neural foraminal narrowing. Ligamentum flavum hypertrophy is noted at this level.

Left and right postero-lateral and far lateral disc herniations with peridiscal osteophytes are noted at L3-L4 level.


The L4-L5 and L5-S1 facet joints show hypertrophic degenerative changes.

A postero-central disc herniation is noted at the L2-L3 level.

A posterior disc bulge with peridiscal osteophytes is noted at the L1-L2 level.

Type II degenerative marrow changes are noted adjacent to the L3-L4, L4-L5 and L5-S1 intervertebral discs.

The rest of the lumbar vertebral bodies reveal normal signal intensity. The visualized pre and paravertebral soft tissues are unremarkable.

The conus medullaris terminates at the L1-L2 level and the thecal sac terminates at the S2 level.

The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :

12.0 mm at L1-L2
13.0 mm at L2-L3
17.0 mm at L3-L4
12.0 mm at L4-L5
10.0 mm at L5-S1.

IMPRESSION :

1. Posterior and bilateral far lateral disc herniations with peridiscal osteophytes at the L5-S1 level.

2. A posterior and left far lateral disc herniation with peridiscal osteophytes at the L4-L5 level. Ligamentum flavum hypertrophy with a tight canal is seen at this level.

4. A postero-central disc herniation at the L2-L3 level.

5. A posterior disc bulge with peridiscal osteophytes at the L1-L2 level.

6. Hypertrophic facetal arthropathy at the L4-L5 and L5-S1 levels.
