Name of the Patient : Abc Xyz Blmn / M / 36 yrs.
Referred by : Dr. Abc Xyz Sarode.
Examination : M.R.I. of the Lumbo-sacral Spine.
CLINICAL PROFILE :
C/O paresthesias in BLE on walking and standing since 2 years.
EXAMINATION :
M.R.I of the lumbo-sacral spine was performed using the following parameters :
5 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
There is slight loss of water content of the L4-L5 and L5-S1 intervertebral discs.
There is a posteriorly herniated disc with peridiscal osteophytes at the L4-L5 level with indentation on the anterior dural theca and bilateral neural foraminal narrowing. Facetal hypertrophy is noted at this level with canal stenosis.
Small postero-central protruded disc is noted at the L5-S1 level.
A posterior and left far lateral disc bulge is noted at the L3-L4 level with left neural foraminal narrowing with indentation upon the foraminal L3 nerve root. Facetal hypertrophy is noted at this level.
A small left far lateral disc bulge is noted at the L2-L3 level with indentation upon the extraforaminal left L2 nerve root.
The facet joints at L2-L3 and L5-S1 levels also appear slightly hypertrophied.
The pedicles of the lower lumbar vertebrae appear congenitally short in their antero-posterior dimensions.
A hemangioma with fat content is noted in the L3 vertebral body.
The rest of the lumbar vertebral bodies and the remaining intervertebral discs reveal normal signal intensity. The visualized pre and paravertebral soft tissues are unremarkable.
The conus medullaris terminates at the L1 level and the thecal sac terminates at the S2 level.
The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :
14.0 mm at L1-L2
15.0 mm at L2-L3
12.0 mm at L3-L4
7.0 mm at L4-L5
9.0 mm at L5-S1.
IMPRESSION :
1. A posteriorly herniated disc with peridiscal osteophytes at the L4-L5 level with facetal hypertrophy and canal stenosis at this level .
2. Small postero-central protruded disc at the L5-S1 level.
