Name of the Patient : Abc Xyz Balmn / F / 60 yrs.
Referred by : Dr. Abc Xyzshru.
Examination : M.R.I. of the Brain.
CLINICAL PROFILE :
C/O headaches and giddiness since 1 year.
EXAMINATION :
M.R.I of the brain was performed using the following parameters :
5 mm thick T1 Weighted, proton and T2 Weighted axial images.
5 mm thick FLAIR and Fast Scan (T2 *) coronal images.
OBSERVATION :
There are ill-defined, hyperintense areas on the proton, T2 Weighted and FLAIR images in the posterior parietal periventricular white matter bilaterally and in the subcortical and deep white matter in the fronto-parietal regions bilaterally.
These lesions appear iso to hypointense on the T1 Weighted images.
Lacunar infarcts are noted in the lentiform nuclei bilaterally.
Both the lateral, third and the fourth ventricles are normal. The basal cisternal spaces are unremarkable. There is slight prominence of the cerebral cortical sulci and cerebellar folia bilaterally. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.
Incidentally noted is an empty sella.

IMPRESSION :
1. Altered signal in the posterior parietal periventricular white matter bilaterally and in the subcortical and deep white matter in the fronto-parietal regions bilaterally most likely represent ischemic changes.
2. Lacunar infarcts in the lentiform nuclei bilaterally.
3. Empty sella.
