CLINICAL PROFILE :
C/O abdominal pain since 4 months.
EXAMINATION :
MR Cholangiogram was performed. 7 mm thick T1 Weighted and T2 Weighted axial images. 5 mm thick T2 Weighted coronal images.OBSERVATION :
There is cylindrical dilatation of the common bile duct from the level of its bifurcation for approximately a distance of 2.6 cms in its extrahepatic portion. The maximum width of this dilatation is 1.1 cms. The cystic duct is seen to insert into this dilated segment. Slight dilatation at the level of the bifurcation of the left hepatic duct is noted.
There is tapering off, of the distal portion of the common bile duct. No filling defects are noted.
Both the visualized kidneys, pancreas, adrenals and spleen are normal in size and shape.The gall bladder is well-distended and does not reveal any intrinsic abnormality and appears folded upon itself.
IMPRESSION :
The MRI features are suggestive of cylindrical dilatation of the common bile duct from the level of its bifurcation upto a distance of 2.6 cms in its extrahepatic portion and would represent a choledochal cyst.
