Name of the Patient : Abc Xyzl lmn / F / 29 yrs.
Referred by : Dr. Abc Xyzcha.
Examination : M.R.I. of the Lumbo-sacral Spine.
CLINICAL PROFILE :
C/O backache with pain radiating to the RLE with paresthesias.
EXAMINATION :
M.R.I of the lumbo-sacral spine was performed using the following parameters :
5 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
There is loss of water content of the L3-L4, L4-L5 and L5-S1 intervertebral discs, with reduction in height of the L3-L4 intervertebral disc.
There is a fairly large posteriorly extruded disc at the L3-L4 level with thecal sac compression. Slight superior and inferior migration of the disc is noted.
Small postero-central protruded discs with peridiscal osteophytes are noted at the L4-L5 and L5-S1 levels.
There is slight forward translation of the L5 over the S1 vertebra with spondylolysis of L5 bilaterally.
There is an increase in the epidural fat in the lumbar canal over the L4 to S1 vertebral levels without significant thecal sac compression.

Slight facetal hypertrophy is noted in the lumbar region.
Type II degenerative marrow changes are noted adjacent to the L3-L4 intervertebral discs.
The rest of the lumbar vertebral bodies and the remaining intervertebral discs reveal normal signal intensity. The visualized pre and paravertebral soft tissues are unremarkable.
The conus medullaris terminates at the L1 level and the thecal sac terminates at the S2 level.
The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :
20.0 mm at L1-L2
20.0 mm at L2-L3
10.0 mm at L3-L4
16.0 mm at L4-L5
17.0 mm at L5-S1.
IMPRESSION :
1. A fairly large posteriorly extruded disc at the L3-L4 level with slight superior and inferior migration of the disc.
2. Slight forward translation of the L4 over the L5 with spondylolysis at L5 with small postero-central protruded discs with peridiscal osteophytes at the L4-L5 and L5-S1 levels.
