Name of the Patient : Abc Xyzkhbhai lmn / M / 60 yrs.
Referred by : Dr. Abc Xyzhru.
Examination : Intracranial and Neck M.R.A.

CLINICAL PROFILE :

C/O paresthesias in the RUE since 15 days.
C/O loss of vision on the left side since 10 years.
Known diabetic.

EXAMINATION :

The brain was screened with 5 mm thick T2 Weighted axial images.

Intracranial and neck MRA were performed with 3D TOF and 2D TOF sequences, respectively.

OBSERVATION :

There is no focal area of altered signal intensity within the brain parenchyma.

The left cerebellar hemisphere appears slightly hypoplastic when compared to the right with a prominent cisterna magna and retrocerebellar cistern.

There is mild dilatation of both the lateral ventricles.
The third and the fourth ventricles are normal. The rest of the basal cisternal spaces are unremarkable. There is slight prominence of the cerebral cortical sulci bilaterally. There is no shift of the midline structures.

Incidentally noted is an empty sella.

INTRACRANIAL MRA :

There is slight irregularity of the midsegment of the left middle cerebral artery with good visualization of the Sylvian segments of the left middle cerebral artery. The A1 segment of the left anterior cerebral artery is hypoplastic.

The petrous, cavernous and supraclinoid segments of the internal carotid arteries bilaterally show normal signal and calibre. The visualized right anterior cerebral, right middle cerebral, basilar, vertebral and posterior cerebral arteries also show normal signal, calibre and wall margins. No obvious aneurysm or vascular malformation is identified.

NECK MRA :

The left cerebellar hemisphere appears slightly hypoplastic when compared to the right with a prominent cisterna magna and retrocerebellar cistern.

The left common carotid artery has a proximal bifurcation as compared to the right.

The common carotid arteries and their bifurcations and the vertebral arteries are unremarkable on either side. There are no vessel wall irregularities or stenosis of the vessels noted.

IMPRESSION :

1. Slight hypoplasia of the left cerebellar hemisphere when compared to the right with a prominent cisterna magna and retrocerebellar cistern.

2. Slight irregularity of the midsegment of the left middle cerebral artery may be due to atherosclerotic changes.

3. No other significant abnormality is detected on the intracranial and neck MRA on this study.
