Name of the Patient : Abc Xyzn Hlmn / M / 19 yrs.
Referred by : Dr. Abc Xyzhrivastav (KEM Hospital).
Examination : M.R.I. of the Left Humerus.
CLINICAL PROFILE :
Known C/O round cell tumor of the left humerus for which patient received chemotherapy.
C/O pain in the left shoulder.
EXAMINATION :
M.R.I of the left humerus was performed using the following parameters :
5 mm thick T1 Weighted and T2 Weighted (with fat saturation) axial images.
4 mm thick T1 Weighted and T2 Weighted (with fat saturation) sagittal images.
4 mm thick T1 Weighted and STIR coronal images.
OBSERVATION :
There is seen an ill-defined, hypointense lesion on the T1 Weighted images in the metadiaphysis of the proximal left humerus. This lesion appears hyperintense on the T2 Weighted and STIR images. The zone of transition between this lesion and the normal marrow is well-defined. Periosteal elevation is noted at the site of the lesion. The cortex appears thickened at the site of the lesion. Susceptibility artifacts in the region of the left deltoid muscles is due to the previous surgery. Scar of previous biopsy is noted along the lateral margin of the proximal left humerus. The lesion extends for about 5.5 cms distally from the epi-metaphyseal junction.
The epiphysis of the left humeral head and greater and lesser tronchanter seem to be spared.
No soft tissue component is identified (except for the periosteal elevation and post-operative changes.
IMPRESSION :
In a known C/O round cell tumor of the proximal left humerus, the MRI features suggest an altered signal intensity lesion in the proximal metadiaphyseal region of the left humerus with periosteal elevation. The supero-inferior extent of the lesion is about 5.5 cms.
The patient is status post-biopsy and post-chemotherapy.
No previous radiographs/investigations were available for review.
