Examination : M.R.I. of the Lumbo-sacral Spine.
CLINICAL PROFILE :
C/O backache with pain radiating to BLE with paresthesias.
EXAMINATION :
M.R.I of the lumbo-sacral spine was performed using the following parameters :
5 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
There is slight loss of water content of the lumbar intervertebral discs.
There is sacralization of the L5 vertebra which is as marked on the film.
Minimal posterior disc bulges are noted at the L3-L4 and L4-L5 levels.
There is slight facetal hypertrophy at the L4-L5 level bilaterally.
The lumbar vertebral bodies are more hypointense than normal on the T1 Weighted images (? preponderance of hematopoeitic marrow). The rest of the facet joints and the visualized pre and paravertebral soft tissues are unremarkable.

The conus medullaris terminates at the L1 level and the thecal sac terminates at the S1 level.
The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :
14.0 mm at L1-L2
14.0 mm at L2-L3
14.0 mm at L3-L4
11.0 mm at L4-L5
9.0 mm at L5-S1.
The sacro-iliac joints were screened with 4 mm thick T1 Weighted and STIR coronal images. There is a 1.4 x 2.1 cms sized hypointense lesion on the T1 Weighted images in the left iliac bone anterior to the left sacro-iliac joint. This lesion remains hypointense on the T2 Weighted images. A focal fatty deposit is noted in the right iliac bone, posteriorly.
The sacro-iliac joints per se are unremarkable.
IMPRESSION :
1. Sacralization of the L5 vertebra which is as marked on the film.
2. Minimal posterior disc bulges at the L3-L4 and L4-L5 levels.
3. Slight facetal hypertrophy at the L4-L5 level bilaterally.
4. Lesion in the left iliac bone as described is not specific for a single etiology (?? bone island). The sacro-iliac joint spaces per se are unremarkable.
