Name of the Patient : Abc Xyza M. Thlmn / M / 57 yrs.
Referred by : Dr. Abc Xyzlkaka.
Examination : Intracranial and Neck M.R.A.

CLINICAL PROFILE :

C/O diminished vision of the right eye since 00.00.00.
Known hypertensive.

EXAMINATION :

The brain was screened with 5 mm thick T2 Weighted axial images.

Intracranial and neck MRA were performed with 3D TOF and 2D TOF sequences, respectively.

OBSERVATION :

Lacunar infarcts (near isointense to CSF on the T2 Weighted images) are noted in the pons, centrally, left anterior capsular region, left thalamus and left deep parietal region.

Prominent perivascular spaces are seen within both cerebral hemispheres.

Both the lateral, third and the fourth ventricles are normal. The basal cisternal spaces are unremarkable. There is no shift of the midline structures.

INTRACRANIAL MRA :

There is concentric narrowing of the cavernous segment of the left internal carotid artery

The petrous, cavernous and supraclinoid segments of the right internal carotid artery and the petrous and supraclinoid segments of the left internal carotid artery show normal signal and calibre. The visualized anterior cerebral, middle cerebral, basilar, vertebral and posterior cerebral arteries also show normal signal, calibre and wall margins. No obvious aneurysm or vascular malformation is identified.

NECK MRA :

There is a concentric narrowing of the proximal 1.0 cm of the right internal carotid artery just distal to the right common carotid bifurcation with resultant mild stenosis of the vessel.

The left common carotid artery and its bifurcation and the vertebral arteries bilaterally are unremarkable.

IMPRESSION :

1. Lacunar infarcts in the pons, centrally, left anterior capsular region, left thalamus and left deep parietal region.

2. Concentric narrowing of the cavernous segment of the left internal carotid artery and proximal 1.0 cm of the right internal carotid artery just distal to the right common carotid bifurcation may be due to atherosclerotic changes. There is resultant mild stenosis of these vessels.
