Name of the Patient : Abc Xyz Islmn / M / 36 yrs.
Referred by : Dr. Abc Xyzoshipura.
Examination : M.R.I. of Both Hips.
CLINICAL PROFILE :
C/O pain in the right hip joint with limp since 4 months and mild fever.
EXAMINATION :
M.R.I of both hips was performed using the following parameters :
5 mm thick T1 Weighted and STIR coronal images.
5 mm thick T1 Weighted and T2 Weighted (with fat saturation) axial images.
5 mm thick Proton density sagittal images.
OBSERVATION :
There is a focal hypointense lesion on the T1 Weighted images in the supero-lateral quadrant of the right femoral head. This lesion appears hyperintense on the T2 Weighted and STIR images.
There is subtle marrow signal change in the head and the neck of the right femur which appears hypointense on the T1 Weighted images and hyperintense on the T2 Weighted and STIR images. Similar areas are also noted in the acetabular portion of the right iliac bone, right ischium and the proximal superior pubic ramus. There is effusion within the right hip joint.
Altered signal intensity is noted in the right gluteus minimus muscle and in the right obturator externus muscle is suggestive of inflammatory edema.
The muscles surrounding the right hip joint appear atrophied as compared to the left.

The femoral head and the acetabulum on the left side reveal normal signal intensity. There is no obvious bony destruction or erosions noted. The articular cartilages are unremarkable.
The musculature surrounding the left hip joint and the visualized pelvis is normal.
IMPRESSION :
The MRI features are suggestive of altered signal intensity within the head and the neck of the right femur and in the right acetabulum with effusion within the right hip joint. This most likely is infective in etiology.
The possibility of this lesion being a neoplastic process is less likely.
