Referred by : Dr. Abc Xyz Mehta.
Examination : M.R.I. of Both Hips.
CLINICAL PROFILE :
C/O pain in the right hip region.
EXAMINATION :
M.R.I of both hips was performed using the following parameters :
5 mm thick T1 Weighted and STIR coronal images.
5 mm thick T1 Weighted and T2 Weighted (with fat saturation
axial images.
5 mm thick Proton density sagittal images.
OBSERVATION :
There is seen a fat signal intensity area in the anterior and superior portion of the right femoral head. This lesion shows a peripheral hypointense signal on the T1 Weighted images. Partial collapse of the right femoral head is noted and the right femoral neck and the anterior column of the acetabulum show an ill-defined hypointense signal on the T1 Weighted images which appears hyperintense on the T2 Weighted and STIR images. A small right hip joint effusion is noted. The articular cartilage overlying the right femoral head is slightly irregular. The muscles around the right hip joint appear atrophied as compared to the left.
The visualized left hip joint is unremarkable.
Multiple small hypointense areas on the STIR images are seen in the myometrium of the uterus and may represent small fibroids. The uterus is slightly bulky.
IMPRESSION :
1. Altered signal intensity lesion in the anterior and superior quadrants of the right femoral head suggests Class A avascular necrosis of the right femoral head.
2. Altered signal in the neck of the right femur and the pubic component of the right acetabulum would represent bone edema/transient osteoporosis.
3. A small right hip joint effusion.
4. Atrophy of the muscles around the right hip joint.
