Name of the Patient : Abc XyzLatif Shlmn / M / 80 yrs.
Referred by : Dr. Abc Xyzlkaka.
Examination : M.R.I. of the Brain.

CLINICAL PROFILE :

C/O frontal headaches with blackouts since 2-4 months.

EXAMINATION :

M.R.I of the brain was performed using the following parameters :

5 mm thick T1 Weighted, proton and T2 Weighted axial images.

5 mm thick FLAIR coronal images.

OBSERVATION :

There are hyperintense areas on the proton, T2 Weighted and FLAIR images in the periventricular deep white matter and are probably ischemic in etiology. These areas appear iso to hypointense on the T1 Weighted images.

There is mild dilatation of both the lateral and third ventricles. The fourth ventricle is normal. There is slight prominence of the cerebral cortical sulci and cerebellar folia bilaterally.

The basal cisternal spaces are unremarkable. There is no shift of the midline structures. The vertebro-basilar system is ectatic. Both eyes appear aphakik.

IMPRESSION :

1. Altered signal in the periventricular deep white matter is
probably ischemic in etiology.

2. Cerebral and cerebellar atrophy.
