Name of the Patient : Abc XyzSlmn / F / 16 yrs.
Referred by : Dr. Abc XyzV. Shah.
Examination : M.R.I. of the Left Knee Joint.

CLINICAL PROFILE :

Alleged H/O fall while playing Kabaddi 3 months back with injury to the left knee joint.
C/O pain, swelling and click in the left knee joint since then.

EXAMINATION :

M.R.I. of the left knee joint was performed using the following parameters :

2 mm thick 3D SPGR sagittal images.

4 mm thick proton density sagittal and axial images with fat saturation.

4 mm thick T1 Weighted and STIR coronal images.

OBSERVATION :

Menisci :

There is a linear hyperintense signal on all the pulse sequences in the posterior horn of the medial meniscus of the right knee joint reaching upto the inferior articular margin. This represents Grade III meniscal signal (meniscal tear).

Similar hyperintense signal is noted in the posterior horn of the lateral meniscus reaching upto the inferior articular surface and this would most likely represent Grade III meniscal signal (meniscal tear).

The anterior horns of the lateral and medial menisci reveal normal configuration and signal characteristics.

Cruciate Ligaments :

The fibres of the anterior cruciate ligament are not visualized. Intermediate signal intensity on the T1 Weighted images which turns hyperintense on the proton density images is seen along the course of the anterior cruciate ligament and most likely represents a strain/tear of the anterior cruciate ligament

The posterior cruciate ligament appears buckled.

Collateral Ligaments and the Patellar Tendon :

An ill-defined hyperintense signal on the STIR images is noted along the course of the deep fibres lateral collateral ligament suggesting a strain.

The medial collateral ligament and the patellar tendon are normal.

Hoffas Fat Pad :

There is streaking of the Hoffas fat pad.

Articular cartilage and bones :

Small marginal osteophytes are seen in the tibia and femur.

Fluid is seen in the left knee joint and suprapatellar recess.

The articular cartilage overlying the patella, tibia and femur appears normal.

IMPRESSION :

1. Tear of the posterior horns of the medial and lateral menisci.

2. Strain/tear of the anterior cruciate ligament.

3. Strain of the lateral collateral ligament.

4. Left knee joint effusion.
