Name of the Patient : Abc XyzVlmn / M / 56 yrs.
Referred by : Dr. Abc Xyzvat.
Examination : M.R.I. of the Cervical Spine.

CLINICAL PROFILE :

C/O backache with progressive weakness of all four extremities.

EXAMINATION :

M.R.I. of the cervical spine was performed using the following parameters :

4 mm thick T1 Weighted and T2 Weighted sagittal images.

5 mm thick T1 Weighted and Fast Scan (T2 *) axial images.

OBSERVATION :

There is loss of water content of the cervical intervertebral discs.

There is a large left paracentral disc extrusion with peridiscal osteophytes compressing the cord at the C6-C7 level. There is superior and inferior migration of this disc. Also seen is ligamentum flavum hypertrophy with resultant canal stenosis at this level. The cord at the C6 and C7 vertebral levels reveals a hyperintense signal on the T2 Weighted and Fast Scan images and this may represent cord edema/ischemia/gliosis.

There are postero-central disc herniations with peridiscal osteophytes indenting the cord at the C3-C4 and C4-C5 levels. Also seen is small posterior disc herniation with peridiscal osteophytes at the C5-C6 level.

A mild posterior disc bulge is seen at the C2-C3 level.

There appears to be thickening (? ossification) of the posterior longitudinal ligament over the C3 to C7 vertebral levels.

The C4-C5 and C5-C6 facet joints show hypertrophic degenerative changes. There is facetal hypertrophy at the C3-C4 level. Mild ligamentum flavum hypertrophy is noted in the rest of the cervical region.

The cervical vertebral bodies show fatty marrow changes. The joints of Luschka and the visualized pre and paravertebral soft tissues are unremarkable.

The atlanto-axial region and the cervico-medullary junction are unremarkable.

The dorsal spine was screened with 4 mm thick T2 Weighted sagittal images and is unremarkable.

IMPRESSION :

1. A large left paracentral disc extrusion with peridiscal osteophytes and ligamentum flavum hypertrophy with resultant canal stenosis at the C6-C7 level.

2. Cord signal alteration at the C6 and C7 vertebral levels may represent cord edema/ischemia/gliosis.

3. Postero-central disc herniations with peridiscal osteophytes at the C3-C4 and C4-C5 levels.

4. A small posterior disc herniation with peridiscal osteophytes at the C5-C6 level.

5. Hypertrophic facetal arthropathy at the C4-C5 and C5-C6 levels.
