Name of the Patient : Abc Xyz L. Kachlmn / M / 70 yrs.
Referred by : Dr. Abc Xyzpadia.
Examination : M.R.I. of the Brain.
CLINICAL PROFILE :
H/O high grade fever since 3 days.
Known diabetic.
C/O giddiness with fall at 8.00 pm on 00.00.00 with 2-3 episodes of seizures and altered sensorium.
EXAMINATION :
M.R.I of the brain was performed using the following parameters :
5 mm thick T1 Weighted, proton and T2 Weighted axial images.
5 mm thick FLAIR, T2 Weighted and Fast Scan (T2 *) coronal images.
SOME IMAGES SHOW PATIENT MOTION.
OBSERVATION :
There is seen a linear hyperintense signal on the proton and T2 Weighted images in the right lentiform nucleus, posteriorly (scans 102.11, 107.9). This lesion appears nearly isointense to normal grey matter on the T1 Weighted images and most likely represents an ischemic focus.
Smaller similar lesions are seen in the head of the left caudate nucleus and periventricular region on the left side and centrum semiovale.
There is mild fullness of both the lateral ventricles. The third and the fourth ventricles are normal. The basal cisternal spaces are unremarkable. There is slight prominence of the cerebral cortical sulci and cerebellar folia bilaterally. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.
There is no obvious evidence of haemorrhage on this study.

IMPRESSION :
Altered signal in the right lentiform nucleus, posteriorly, head of the left caudate nucleus, periventricular region on the left side and centrum semiovale most likely represent ischemic lesions.
No other significant abnormality is detected on this study.
