Name of the Patient : Abc Xyza Tlmn / F / 20 yrs.
Referred by : Dr. Abc Xyzhah.
Examination : M.R.I. of the Dorsal Spine.
CLINICAL PROFILE :
C/O backache with slight weakness of BLE since 15 days.
H/O laminectomy of the D4/D5 vertebrae with anterior decompression of L1/L2 in 0000.
EXAMINATION :
M.R.I of the dorsal spine was performed using the following parameters :
4 mm thick T1 Weighted and T2 Weighted sagittal images.
5 mm thick T1 Weighted and T2 Weighted axial images.
OBSERVATION :
There are post-operative changes in the posterior soft tissues over the D5 to D7 vertebral levels and laminectomy of the D5, D6 and D7 vertebral bodies.
The dorsal spinal cord over the D4 to D8 vertebral levels appear to be reduced in its antero-posterior dimensions. A subtle hyperintense signal is seen within the spinal cord at the D6 and D7 vertebral levels which is isointense to normal cord on the T1 Weighted images and would represent myelomalacia, in the given clinical setting.
The visualized dorsal vertebral bodies and intervertebral discs reveal normal signal intensity. The facet joints and the visualized pre and paravertebral soft tissues are unremarkable.
The conus medullaris terminates at the L1 level.
The cervico-dorsal spine was screened with 4 mm thick T2 Weighted sagittal images which shows slight reduction in the antero-posterior dimensions of the spinal cord.
The lumbar spine was screened with 5 mm thick T1 Weighted sagittal images which shows wedging of the L2 vertebral body with reduction in the L2-L3 disc space and partial fusion of the L2 and L3 vertebral bodies. Fatty changes are noted in the L2 and L3 vertebral bodies indicating healing. Slight forward translation of the L4 over the L5 vertebral body is noted.
IMPRESSION :
1. Post-operative status.
2. Thinning of the dorsal spinal cord in its antero-posterior dimensions over the D4 to D8 vertebral levels with altered cord signal within the spinal cord at the D6 and D7 vertebral levels represent myelomalacia changes.
3. Wedging of the L2 vertebral body with partial fusion of the L2 and L3 vertebral bodies and fatty changes is suggestive of healing.
